The registry will track clinical outcomes and episodes of care across the participating patient populations, tracks patient engagement and defines patient treatment status across the clinical caseload, prompts measurement-based treatment to target (including appointment and referral reminders), facilitates efficient, systematic psychiatric caseload review and monitoring of individual patient progress, has patient-level reporting capability (including contact attempts, medications, and PHQ-9 and GAD-7 scores), and is securely accessible by partnering CoCM psychiatric consultants.
