The Collaborative Care Model (CoCM)

  • What is the Collaborative Care Model (CoCM)

    The Collaborative Care Model is an evidence-based approach where a primary care provider, behavioral health care manager, and consulting psychiatrist work together as a team to manage patients’ behavioral health needs.

  • How does CoCM work in practice

    The care manager supports patients directly, tracks progress using validated tools, and consults regularly with a psychiatric provider who advises the primary care team on treatment adjustments.

  • What are the benefits of CoCM

    CoCM leads to better outcomes for depression, anxiety, and other conditions, improves provider confidence, and allows for more efficient use of psychiatric resources through indirect consultation.

    The Hub offers scheduled case reviews, same-day provider-to-provider consultations, and educational case discussions focused on evidence-based management of behavioral health conditions.

  • Who can access the Psychiatric Hub

    Partner clinics and spoke sites participating in the UPHCS Behavioral Health Network can access the Hub for both patient-specific consultations and programmatic support.

    The Psychiatric Hub provides consultative psychiatric support for primary care and CoCM teams across the Upper Peninsula, along with Emergent Care providers. It helps bridge access gaps by offering case review, diagnostic clarification, and treatment recommendations.

  • What does it mean to be a UPHCS Behavioral Health Spoke

    A Spoke is a participating clinic or site that partners with the UPHCS Behavioral Health Hub to integrate Psychiatric Consultation and Collaborative Care services within their own practice.

  • What support do Spokes receive

    Spokes receive access to psychiatric consultation, training on behavioral health integration and billing, workflow development support, and connection to community and referral resources through our BH Collaborative.

  • How can my organization become a Spoke site

    Organizations interested in becoming a Spoke can contact UPHCS to learn about participation requirements, implementation steps, and available support for onboarding and training.

How a Provider Uses the Service: Step-by-Step

  • 1. Identify Behavioral Health Needs

    Use universal screening tools during routine visits (PHQ-9, GAD-7, etc.). 

    You continue leading care. The Hub strengthens your decision-making.  

  • 2. Access Psychiatric Consultation

    Need input on diagnosis, medication adjustments, or treatment planning?  

    • Submit a consultation request 
    • The Hub schedules a provider consultation phone call or via TigerConnect
    • Consulting psychiatrist provides verbal and written recommendations to provider  
  • 3. Psychiatric Consultation for Collaborative Care (CoCM)

    The UPHCS Behavioral Health Hub provides dedicated psychiatric consultation for practices implementing the Collaborative Care Model (CoCM).   

    Practices receive:  

    • A consistent psychiatric consultant 
    • Clinical guidance that supports compliant CoCM billing 
    • An affordable, CMS-aligned patient registry built for rural practices 
    • A CMS-aligned patient registry required for CoCM 

    The UPHCS registry was developed specifically to meet the operational needs and budgets of rural practices. 

    For practices beginning CoCM, the Hub can connect practices to implementation resources. Participation in Provider Organizations is not required to partner with the Hub. Through this shared model, practices gain required psychiatric infrastructure at significantly lower cost than hiring or contracting independently.   

    We help you build and sustain integrated care – not just start it.  

  • 4. Coordinate Direct Services When Needed

    If a patient requires higher-level care:

    • The Hub assists with specialty referral coordination  
    • Qualifying pediatric patients referred to Michigan Consultation & Care (MC3)
    • Qualifying patients with OUD/SUD referred to Great Lakes Recovery Center (GLRC)
    • You receive guidance on appropriate referral pathways
    • Transitions are streamlined
